About the Role

The Licensed Practical Nurse (LPN) is a key member of our care team, responsible for coordinating care delivery and ensuring the highest quality of life for our residents. As a team leader, you will be responsible for assessing residents' needs, implementing and evaluating individualized care plans, and collaborating with other disciplines to ensure seamless care delivery.

Responsibilities
  • Assess residents' needs and develop personalized care plans to meet their unique requirements.
  • Liaise with physicians, families, and other healthcare professionals to ensure comprehensive care.
  • Administer medications and treatments as prescribed, and monitor residents' physical and psychological conditions.
  • Participate in interdisciplinary care conferences to ensure resident-centered care.
  • Support the administrative requirements of the community, including admissions and confidentiality maintenance.
  • Provide coaching and direction to care staff, and participate in orientation and education programs.
  • Maintain up-to-date knowledge of community policies and procedures, and perform other related duties as required.
Qualifications and Skills
  • Graduate of an approved Practical Nursing program and current licensure with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
  • Valid Occupational First Aid Level 1 certification.
  • Successful annual Resident Assessment Instrument (RAI) competency testing.
  • Previous experience in a long-term care facility an asset.
  • Ability to read, write, and speak English proficiently, and understand verbal and written instructions.
  • Computer skills, including basic navigation of Word, Excel, and Outlook.
  • Must be in good health, physically able to perform repetitive tasks, lift, squat, bend, stand, and walk as required.
