Job Summary

We are seeking a highly skilled Care Coordinator to join our team at Ontario Health atHome. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs, providing access to community services, and engaging with patients, caregivers, and healthcare practitioners.

Key Responsibilities
  • Assess and Determine Patient Care Needs
    • Collaborate with patients and their families to assess care needs and determine eligibility for services
    • Develop individual care and service plans that meet the unique needs of each patient
  • Provide Access to Community Services
    • Link patients with service providers and community resources
    • Coordinate and monitor care plan delivery to ensure seamless transitions and optimal outcomes
  • Engage with Patients, Caregivers, and Healthcare Practitioners
    • Establish a helping relationship with patients and their families, built on trust, empathy, and effective communication
    • Collaborate with healthcare practitioners to ensure comprehensive and coordinated care
Requirements
  • Education and Experience
    • Membership, in good standing, with the applicable regulatory body (e.g. College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.)
    • 2+ years of recent experience in community health or a related field
  • Skills and Abilities
    • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
    • Effective time management, prioritization, and organizational skills, with the ability to work independently and collaboratively in a busy multidisciplinary environment
    • Established ability to accurately complete required documentation, reports, and forms
  • Additional Requirements
    • A valid driver's license and access to a reliable vehicle
    • Proficient in a Windows environment
    • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.
Preferred Qualifications
  • Experience Working with Diverse Patient Groups
    • Experience working with multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, and pediatric patient populations
  • Case Management Experience or Recent Related Community Experience
    • Experience in case management or recent related community experience
  • Language Skills
    • Ability to speak French or another second language
