The Clinical Coordinator plays a pivotal role in the management of the Transitional Care Unit (TCU) at Southlake Regional Health Centre. This position is responsible for coordinating patient flow, bed utilization, and resource management, ensuring seamless care delivery.
Key Responsibilities- Liaise with physicians, nurses, and other healthcare professionals to ensure effective communication and collaboration.
- Coordinate patient transfers and bed assignments to optimize resource utilization.
- Lead the team in implementing evidence-based practices and promoting a patient-centered care model.
- Perform human resource responsibilities, including recruitment, hiring, and performance evaluations.
- Develop and implement clinical pathways and protocols to enhance patient care and outcomes.
- Support the development of a patient-centered care environment and shared governance models.
- Act as a champion for corporate initiatives, such as hand hygiene and staff/patient satisfaction.
- Participate in the accreditation process and contribute to the development of program goals and objectives.
- Regulated healthcare professional with current registration in good standing.
- Minimum five years of direct patient care experience, preferably in a community healthcare setting.
- Progressive administrative experience, including team leadership and supervision.
- Strong commitment to patient and family-centered care.
- Excellent communication and interpersonal skills.
- Geriatric certificate or equivalent.
