Geriatric Medicine Specialist

**Overview:**
We are seeking a highly skilled Geriatric Medicine Specialist to provide exceptional primary medical care to patients and supervise medical care personnel. This is an outstanding opportunity to join the TEEMA Group team and contribute to delivering high-quality care within an interdisciplinary framework.
Job Description:
- Conduct comprehensive history and physical assessments for new referrals, ensuring accurate diagnosis and effective treatment plans.
- Perform interval history and physical exams for established participants, conducted quarterly or as needed, to monitor progress and adjust care accordingly.
- Evaluate and treat participants with episodic illnesses, utilizing evidence-based practices and staying up-to-date on best treatments.
- Making referrals to Medical Specialists when necessary, prioritizing timely and effective collaboration to ensure seamless care delivery.
- Admit participants to the hospital when appropriate, providing primary care responsibilities during hospitalization and coordinating discharge planning with the IDT to promote smooth transitions.
- Oversee the care of participants residing in nursing homes, including regular visits and communication with nursing home staff to ensure continuity of care.
- Engage in family conferences for care planning and implementation, fostering strong relationships with patients, families, and healthcare teams.
- Coordinate 24-hour care delivery, ensuring uninterrupted support and addressing any concerns promptly.
- Document participant changes accurately in medical records, communicating these updates to the IDT in a timely manner to maintain informed decision-making.
- Participate in the IDT, attend staff meetings, and contribute to participant care planning, driving collaborative efforts to achieve optimal patient outcomes.
- Stay updated on quality/clinical standards and best practices for the geriatric population, applying this knowledge to enhance care delivery and improve patient experiences.
Requirements:
- A valid MD license in good standing to practice medicine in the State of practicing.
- A graduate of an accredited School of Medicine.
- A minimum of two years of clinical experience required, with a preference for candidates with experience working with frail or elderly populations or those willing to undergo training upon hiring.
- Knowledge of geriatric medicine and the special needs of complex geriatric patients, with fellowship or additional training in Geriatrics being desirable.
- An understanding of the culture and needs of socially and ethnically diverse populations, reflecting our commitment to inclusive and compassionate care.
**Estimated Salary Range:** $175,000 - $225,000 per annum, commensurate with experience and qualifications. We offer a competitive compensation package that reflects your value to our team and the importance of attracting top talent in the field of geriatric medicine.
