**Overview:**
The TEEMA Group Solutions team is seeking a dedicated Physical Therapist to provide comprehensive physical therapy services. As a skilled professional, you will be responsible for assessing, diagnosing, and treating physical disabilities, ensuring patients receive high-quality care.
**Key Responsibilities:**
- Evaluate patients to determine physical therapy needs and develop personalized treatment plans.
- Provide physical therapy treatments and educate patients and their families on therapy exercises.
- Monitor patient progress, modify treatments as needed, and document care accurately.
- Ensure therapy adheres to industry standards and explain procedures clearly.
- Coordinate 24-hour care, train family members and staff, and make home visits for necessary interventions.
- Record observations, address variations in care, and participate in Quality Improvement processes.
- Supervise aides and staff, ensuring safe and effective care delivery, and actively participate in interdisciplinary team meetings and care planning.
**Requirements:**
- Bachelor's degree in Physical Therapy or Master's degree in Physical Therapy.
- Knowledge of clinical competency with the geriatric population.
- Current Licensed Physical Therapist in the required state.
- Preferably, 1 year of experience with a frail or elderly population (training provided if not present).
