**Position Overview**
We are seeking an experienced and motivated **Casual Discharge Planner** to join our team. In this role, you will play a critical part in
facilitating timely and effective discharges for patients, ensuring their care transitions seamlessly from hospital to home or other
appropriate settings. This position reports to the Clinical Director of Medical, Rehab, Nurse Practitioners and Patient Flow.

**Key Responsibilities**:

- Collaborate with interdisciplinary teams to develop and implement discharge plans tailored to individual patient

needs.
- Utilize tools such as WTIS (Wait Time Information System) and ALCIS (Alternate Level of Care Information System)

to optimize discharge efficiency.
- Serve as a liaison between the hospital, patients, families, and community resources to ensure continuity of care.
- Assess patient needs and connect them with appropriate community services, supports, and follow-up care.
- Maintain accurate documentation and comply with hospital policies and regulatory requirements.

**Qualifications**:

- Current registration in good standing as a Registered Nurse with the College of Nurses of
- Minimum of 5 years’ experience as a Registered Nurse in acute care or long-term care settings.
- Previous experience in discharge planning within a hospital or healthcare setting.
- Familiarity with WTIS, ALCIS, and a strong understanding of local and regional community resources.
- Exceptional organizational and communication skills, with the ability to work effectively in a fast-paced

environment.
- Strong critical thinking and problem-solving abilities.
- Degree or diploma in a relevant field (e.g., Nursing, Social Work, or a related healthcare discipline) preferred.
- Commitment to the mission and values of NGH.
