Responsibilities- Conduct comprehensive history and physical assessments for new referrals, evaluating each patient's unique needs and circumstances.
- Perform interval history and physical exams for established participants, ensuring timely and accurate monitoring of their health.
- Evaluate and treat participants with episodic illnesses, providing prompt and effective care to address their medical concerns.
- Make referrals to Medical Specialists as necessary, collaborating with other healthcare professionals to ensure comprehensive care for our patients.
- Admit participants to the hospital when appropriate, providing primary care responsibilities during hospitalization and coordinating discharge planning with the IDT.
- Oversee the care of participants residing in nursing homes, including regular visits and communication with nursing home staff to ensure seamless care coordination.
- Engage in family conferences for care planning and implementation, working closely with patients and their loved ones to develop personalized care plans.
- Coordinate 24-hour care delivery, ensuring that our patients receive the care they need at all times.
- Document participant changes accurately in medical records and communicates these changes to the IDT promptly, maintaining open lines of communication and ensuring timely updates.
- Participate and attend staff meetings, contributing to participant care planning and staying up-to-date on quality/clinical standards and best practices for the geriatric population.
- A valid MD/DO license in good standing to practice medicine in the State of practicing.
- Board Certification.
- Graduate of an accredited School of Medicine.
- A minimum of two years of clinical experience required.
- Experience working with a frail or elderly population preferred, or willingness to undergo appropriate training upon hiring.
- Knowledge of geriatric medicine and the special needs of complex geriatric patients.
- Fellowship or additional training in Geriatrics preferred.
- Understanding of the culture and needs of socially and ethnically diverse populations.
