We are seeking a skilled Health Care Social Worker to join our team at the Pain Clinic in Yarmouth Regional Hospital. As a member of our interprofessional team, you will play a vital role in delivering Primary Health Care and chronic disease management care, with a focus on chronic pain.
The successful candidate will be responsible for building relationships with patients and families, assessing their needs, and developing and implementing plans to address their psychosocial, emotional, and spiritual well-being. You will work collaboratively with our team to provide dynamic and interactive services, focusing on the strengths of patients and clients, and promoting their coping strategies and problem-solving skills.
We are looking for a candidate with a Master's degree in social work from a recognized university, current registration with the Nova Scotia College of Social Workers, and at least 2 years of post-MSW clinical experience. Previous experience working in Primary Care and/or community-based settings is an asset.
The ideal candidate will possess excellent communication skills, the ability to work independently and collaboratively, and a demonstrated commitment to diversity, equity, and inclusion. They will also have the ability to work flexibly and creatively to provide services, and be comfortable working with an interdisciplinary team and community partners.
Key Responsibilities:
- Assess patients' and families' needs and develop plans to address their psychosocial, emotional, and spiritual well-being
- Build relationships with patients and families, and work collaboratively with the team to provide dynamic and interactive services
- Focus on the strengths of patients and clients, and promote their coping strategies and problem-solving skills
- Work independently and collaboratively as part of an interdisciplinary team
- Develop and maintain knowledge of local community resources and agencies
Requirements:
- Master's degree in social work from a recognized university
- Current registration with the Nova Scotia College of Social Workers
- At least 2 years of post-MSW clinical experience
- Previous experience working in Primary Care and/or community-based settings is an asset
