Healthcare Social Worker

We are seeking a skilled Health Care Social Worker to join our interdisciplinary team at the Yarmouth Regional Hospital. As a member of our team, you will play a vital role in the delivery of Primary Health Care and chronic disease management, with a focus on chronic pain.
The successful candidate will be responsible for building upon the strengths of patients/clients, focusing on existing coping strategies and problem-solving skills, past successes, and personal resources. They will work collaboratively with an interdisciplinary team to provide dynamic and interactive processes of engagement, assessment, planning, intervention, and evaluation.
The ideal candidate will possess a Master's degree in social work from a recognized university, current registration with the Nova Scotia College of Social Workers, and 2 years of post-MSW related clinical experience or relevant work experience. They will also have demonstrated ability to use a client-/family-centered approach, psychotherapy skills, and familiarity with various evidence-based treatment approaches.
We offer a competitive hourly rate of $37.31 - $49.04 and a comprehensive benefits package, including health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
About You- Master's degree in social work from a recognized university
- Current registration with the Nova Scotia College of Social Workers
- 2 years post MSW related clinical experience or relevant work experience required
- Previous experience working in Primary Care and/or community-based setting preferred
- Demonstrated ability to use a client-/family-centered approach
- Psychotherapy skills (i.e. ACT and CBT)
- Familiarity with various evidence-based treatment approaches including Motivational Interviewing and behaviour change counselling
- Demonstrated experience with group facilitation
- Ability to complete biopsychosocial assessments
- Excellent communication skills
- Demonstrated ability to assist patients and families with navigation of local community resources and agencies
- Ability to demonstrate effective time management and disciplined self-organization
- Demonstrated comfort and skills working collaboratively with an interdisciplinary team and community partners
- Demonstrated ability to communicate effectively with patients, families, public, community partners, primary care team members and other professionals and agencies using verbal, written and computer communication means
- Demonstrated ability to work both independently and collaboratively within an interdisciplinary team setting
- Demonstrated ability and comfort in working flexibly and creatively to provide services
- Demonstrates commitment to diversity, equity and inclusion
- Awareness of local community resources and agencies would be considered an asset
- Valid Nova Scotia Driver's license and access to a reliable vehicle
- Competencies in other languages preferred; French an asset
- Competencies in other languages an asset, French and Mi'kmaw preferred
