About the Opportunity

We are seeking a skilled Health Care Social Worker to join our team at Nova Scotia Health. The successful candidate will play a vital role in the delivery of Primary Health Care, chronic disease management care, with a focus on chronic pain.

The Health Care Social Worker will be a member of a highly collaborative interprofessional team, working closely with patients, families, and community partners to provide comprehensive care. The ideal candidate will have a strong understanding of the social determinants of health and be able to apply this knowledge in a dynamic and interactive process of engagement, assessment, planning, intervention, and evaluation.

The successful candidate will have a Master's degree in social work from a recognized university, current registration with the Nova Scotia College of Social Workers, and at least 2 years of post-MSW related clinical experience. Previous experience working in Primary Care and/or community-based settings is preferred.

  • Ability to work collaboratively with an interdisciplinary team
  • Experience with group facilitation and psychotherapy skills
  • Familiarity with evidence-based treatment approaches
  • Ability to complete biopsychosocial assessments
  • Excellent time management and self-organization skills
Hours and Compensation

This is a permanent full-time position, with 75 hours bi-weekly. The successful candidate will be compensated at a rate of $37.31 - $49.04 per hour.
