The Nova Scotia Health Authority is seeking a skilled Health Care Social Worker to provide high-quality care and support to patients and their families in palliative settings. Job Description

This long-term, full-time position offers the chance to work collaboratively with interdisciplinary teams to develop and implement comprehensive care plans that address the physical, emotional, and social needs of patients and their loved ones.

Key Responsibilities:
  • Conduct thorough psychosocial assessments to identify patient needs and goals
  • Develop and implement personalized care plans that address cognitive, emotional, and spiritual needs
  • Collaborate with healthcare teams to coordinate care and ensure seamless transitions between settings
  • Foster strong relationships with patients, families, and caregivers to promote optimal well-being
Requirements

To be successful in this role, you will possess:

  • A Master of Social Work degree from a recognized university
  • Registration and Membership in the Nova Scotia College of Social Workers and Canadian Association of Social Workers
  • Minimum two years of clinical experience as a Social Worker, preferably in palliative care
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
  • A strong understanding of human behavior theories, including systems theory, family dynamics, and group dynamics
What We Offer

We offer a competitive salary range of $73,662 - $97,528 per year, depending on qualifications and experience.
