Community Outreach Specialist

We are seeking an experienced Community Outreach Specialist to join our team in Prince George. This role will provide outreach support to clients referred by our funders and community partners. You will work closely with clients to assess their needs and develop personalized plans to improve housing stability and address related issues.
About the Role
This is a unionized position (CUPE Local 1936-11) with a competitive salary of $27.54 per hour, plus benefits enrollment as per the Collective Agreement. The successful candidate must be legally authorized to work in Canada and commit to flexible working hours, including some evenings/weekends as determined by program/client needs and schedules.
Key Responsibilities
- Conduct referral consultations based on criteria, urgency, and other factors to aid with a successful transfer or referral process
- Work with clients to identify needs and develop personalized plans to improve housing stability and address related issues
- Provide coordinated case management services, physical health, mental wellness, and substance use supports, social, emotional, and community supports, personal care and personal living supports, food security supports, and indigenous cultural supports
- Interact with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client's interest and supportive of long-term goals
- Data collection and input through monthly reports and analysis of case notes
- Required to drive between various sites to meet with clients and community partners, as well as transport individuals between sites or to appointments within the community
Required Skills and Qualifications
- Graduate with a Diploma or Certificate in a relevant field with one year of related experience
- Demonstrated ability to respond well in crisis situations
- Experience working with clients with a history of conflict with the law, developmental disabilities, and/or spectrum disorders, interaction with social agencies, and experiencing psychiatric/personality disorders
- Understanding of providing service to people with different learning abilities
- Extensive knowledge of community services in Northern British Columbia
- Experience in assessment, case/care plan development, and implementation
- Ability to deal with potential violence and develop programs tailored to diverse individual needs
- Commitment to Indigenous Truth and Reconciliation, cultural safety, and decolonization practices
- Excellent written and verbal communication skills in English
