About the Role

This position plays a critical role in improving housing stability through integrated service provision. As an Outreach Support Specialist, you will provide outreach support to clients referred by our funders and community partners. Your primary objective will be to assist vulnerable individuals in maintaining housing in the private rental market.

Duties and Responsibilities

  • Conduct referral consultations based on criteria, urgency, and other factors to aid with a successful transfer or referral process.
  • Work with clients to research background material, seek input from professionals and family members, and provide assessments that ascertain needs and postulate intervention possibilities.
  • Create mutually agreed-upon intervention and support plans with measurable short- and long-term goals.
  • Ensure aspects of daily living, including housing, nutrition, clothing, hygiene, leisure lifestyle, and access to employment, education, and other essential services, are addressed in planning.
  • Provide coordinated case management services, physical health, mental wellness, substance use supports, social, emotional, and community supports, personal care, and personal living supports, food security supports, and Indigenous cultural supports.
  • Offer direct non-clinical supports to recipients and collaborate with community partners, including the health authorities.
  • When necessary, provide a wide range of interventions to support clients in addressing and accomplishing established goals.
  • Interact with other professionals, family members, and organizations to ensure services, admissions, and treatments align with client interests and support long-term goals.
  • Data collection and input through monthly reports and analysis of case notes.
  • Required to drive between various sites to meet with clients and community partners and transport individuals between sites or to appointments within the community.
  • Participate in staff meetings as requested by the Program Manager.

Requirements

  • Graduate with a Diploma or Certificate in a relevant field with one year of related experience.
  • Demonstrate a clear ability to respond well in crisis situations.
  • Experience working with clients with a history of conflict with the law, developmental disabilities, and/or spectrum disorders, interacting with social agencies, and experiencing psychiatric/personality disorders.
  • Understanding of providing service to people with different learning abilities.
  • Extensive knowledge of community services in Northern British Columbia.
  • Experience in assessment, case/care plan development, and implementation.
  • Demonstrated ability to deal with potential violence.
  • Demonstrated ability to develop programs tailored to diverse individual needs.
  • Commitment to Indigenous Truth and Reconciliation, cultural safety, and decolonization practices.
  • Excellent written and verbal communication skills in English.
