Join Nova Scotia Health and IWK Health as a Care Coordinator in our healthcare team. As a member of our interdisciplinary team, you will provide collaborative, holistic care on the primary to quaternary care continuum.

Key Responsibilities:
  • You will be primarily concerned with the relationships between people and their environments, which are integral to psychosocial, emotional and spiritual wellbeing.
  • You will focus on existing coping strategies and problem-solving skills, past successes, and personal resources.

In this role, you will work collaboratively with community partners to deliver high-quality care to patients. You will have the opportunity to work with a variety of clients and develop your skills in client-/family-centered approaches, group facilitation, and biopsychosocial assessments.

Requirements:
  • You must have a Bachelor's degree in Social Work from a recognized university.
  • You must be registered with the Nova Scotia College of Social Workers.
  • You must have at least two years of post-BSW clinical experience.
  • You must have knowledge and understanding of the Diagnostic & Statistical Manual of Mental Disorders.
  • Prior experience working in adult community mental health is preferred.
Working Conditions:
  • This is a permanent full-time position with 75 hours bi-weekly.
  • Shifts will include weekdays, weekends, and holidays.
  • You may be required to travel to other sites.
Compensation:

The estimated annual salary for this role is $70,000-$90,000, based on an hourly rate of $33.96-$42.89. This compensation package includes a comprehensive benefits plan.
