Job Summary:
Home and Community Care Support Services is seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will be responsible for managing a caseload of diverse patients who are living with various health concerns. You will assess patients' care needs, develop and implement care plans, and coordinate the delivery of Home and Community Care services.
Key Responsibilities:
- Conduct comprehensive patient assessments to identify clinical, physical, and psychosocial needs
- Develop and implement individualized care plans that reflect patients' priorities and desired outcomes
- Coordinate the delivery of Home and Community Care services, including medical equipment, supplies, and services
- Facilitate understanding and cooperation among members of the patient's healthcare team
- Provide regular reports to physicians, service providers, patients, and caregivers as required
Requirements:
- Registered Nurse (BScN or diploma) and registered with the College of Nurses of Ontario; or
- Degree or diploma in Physiotherapy and registered with the College of Physiotherapy of Ontario; or
- Degree or diploma in Occupational Therapy and registered with the College of Occupational Therapy of Ontario; or
- Degree in Social Work and registered with the Ontario College of Social Workers and Social Service Workers; or
- Degree in Speech Therapy and registered with the College of Audiologists and Speech-Language Pathologists of Ontario; or
- Degree in Foods and Nutrition or equivalent and registered with College of Dietitians of Ontario
Preferred Qualifications:
- Related professional experience (community-based experience preferred)
- Knowledge of medical diagnoses and prognoses, symptom management, and cognitive/psychiatric disorders
- Knowledge of case management principles and best practices
- Knowledge of relevant health care legislation
What We Offer:
- Competitive salary range: $40.409/hr - $43.169/hr
- Flexible work options, including a hybrid model of work with a combination of community and clinical settings, patients' homes, on-site, and remote
