Job Overview:



We are seeking a skilled Patient Navigator to join our team at Home and Community Care Support Services Champlain. As a Patient Navigator, you will play a vital role in coordinating care for patients with diverse needs.



Key Responsibilities:


  • Conduct comprehensive patient assessments to identify clinical, physical, and psychosocial needs
  • Develop and implement individualized care plans that address patient priorities and desired outcomes
  • Coordinate the delivery of home and community care services to patients and their caregivers
  • Facilitate communication among healthcare providers and stakeholders to ensure seamless care coordination
  • Maintain accurate and up-to-date case documentation in accordance with organizational policies


Requirements:

To be successful as a Patient Navigator, you will require:




  • A degree or diploma in a relevant field (e.g., nursing, physiotherapy, occupational therapy, social work, speech therapy)
  • Related professional experience, preferably in a community-based setting
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment and independently as needed
  • Strong knowledge of medical diagnoses, prognoses, symptom management, and cognitive/psychiatric disorders
  • Familiarity with case management principles and best practices
  • Knowledge of relevant healthcare legislation and policies
