Job Overview:
We are seeking a skilled Patient Navigator to join our team at Home and Community Care Support Services Champlain. As a Patient Navigator, you will play a vital role in coordinating care for patients with diverse needs.
Key Responsibilities:
- Conduct comprehensive patient assessments to identify clinical, physical, and psychosocial needs
- Develop and implement individualized care plans that address patient priorities and desired outcomes
- Coordinate the delivery of home and community care services to patients and their caregivers
- Facilitate communication among healthcare providers and stakeholders to ensure seamless care coordination
- Maintain accurate and up-to-date case documentation in accordance with organizational policies
Requirements:
To be successful as a Patient Navigator, you will require:
- A degree or diploma in a relevant field (e.g., nursing, physiotherapy, occupational therapy, social work, speech therapy)
- Related professional experience, preferably in a community-based setting
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment and independently as needed
- Strong knowledge of medical diagnoses, prognoses, symptom management, and cognitive/psychiatric disorders
- Familiarity with case management principles and best practices
- Knowledge of relevant healthcare legislation and policies
