Job Summary:
Home and Community Care Support Services is seeking a qualified Care Coordinator to join our team. As a Care Coordinator, you will work with patients, families, and healthcare providers to develop and implement care plans that meet the unique needs of each individual.
Key Responsibilities:
- Assess client needs and develop care plans that prioritize their health and well-being
- Collaborate with healthcare providers and community resources to ensure seamless care transitions
- Provide education and support to clients and their families to promote independence and self-management
- Monitor and evaluate care plans to ensure optimal outcomes
- Communicate effectively with clients, families, and healthcare providers to ensure coordinated care
Requirements:
- Registered Nurse (BScN or diploma) and registered with the College of Nurses of Ontario; or
- Degree or diploma in Physiotherapy and registered with the College of Physiotherapy of Ontario; or
- Degree or diploma in Occupational Therapy and registered with the College of Occupational Therapy of Ontario; or
- Degree in Social Work and registered with the Ontario College of Social Workers and Social Service Workers
- Fluency in English and French is required
Working Conditions:
- Monday to Friday, 8:30am to 4:30pm
- Hawkesbury Hospital - 1111 Ghislain St, Hawkesbury ON
