Job Overview:
Home and Community Care Support Services is seeking a skilled Bilingual Care Coordinator to join our team. This role requires the ability to work independently and as part of a team, with excellent communication and interpersonal skills.
Job Summary
This position plays a crucial role in managing a caseload of diverse patients who require care and support services. The successful candidate will be responsible for assessing patient needs, developing individualized care plans, and coordinating services to meet those needs.
Responsibilities
- Conduct comprehensive assessments of patient needs, including clinical, physical, and psychosocial factors
- Develop and implement individualized care plans that address patient goals and priorities
- Coordinate services from contracted providers, including medical equipment, supplies, and community resources
- Communicate effectively with patients, families, caregivers, and healthcare teams to ensure seamless care delivery
- Document patient information and progress in accordance with Home and Community Care Support Services policies
Requirements
To be successful in this role, you will need:
- A degree or diploma in a relevant health profession (e.g., nursing, physiotherapy, occupational therapy, social work)
- Registration with the relevant college or association in Ontario
- At least 2 years of experience in a related field, preferably in a community-based setting
- Excellent communication and interpersonal skills, with the ability to work with diverse populations
- Proficiency in English and French, with the ability to communicate effectively in both languages
Salary and Benefits
The salary range for this position is $40.409-$43.169 per hour, depending on experience. In addition to a competitive salary, we offer a range of benefits, including health and dental insurance, retirement savings plan, and paid time off.
