Job Summary

We are seeking a highly skilled and dedicated Care Coordinator to join our team at Home and Community Care Support Services Champlain. This is an excellent opportunity for a registered healthcare professional to make a meaningful difference in the lives of clients and their families.

About the Role

The successful candidate will work under the general direction of the Manager, Home and Community Care and in accordance with our standards, legislation, policies, and guidelines. Your primary responsibilities will include client assessment, determination of eligibility, admission, service planning, authorization, implementation, monitoring, reassessment, adjustment, and discharge planning of all client service programs (home care and placement).

Key Responsibilities:
  • Identification and Engagement: Assess referred clients for admission and establish a helping relationship with eligible clients and their families while honoring their values and preferences.
  • Assessment: Perform multi-dimensional assessments and identify the need for crisis intervention and/or prevention strategies.
  • Goal Setting and Service Planning: Establish a care and service plan balancing clients' needs, choices, and available resources. Develop a care plan in collaboration with the client, other healthcare professionals, care providers, or family, stating priorities, services authorized, mutual goals, and anticipated duration of service.
  • Accessing Resources and Linking: Collaborate and partner with networks of service providers.
  • Service Implementation: Initiate and authorize the implementation of the care plan.
  • Accountability, Monitoring, and Reassessment: Review and maintain care plans in compliance with quality standards, outcome measures, and quality indicators.
  • Discharge Planning: Conduct discharges from service processes and/or redirect clients and families to alternative resources.
Requirements

To be considered for this role, you must have a degree or diploma in one of the following fields: Registered Nurse (BScN or diploma), Physiotherapy, Occupational Therapy, Social Work, Speech Therapy, or Foods and Nutrition. You must also have at least 2 years of related professional experience and possess fluency in English and French. Additionally, you will require valid N95 Mask Fit Test results or documentation of a medical exception within the past two years and a Vulnerable Sector Check completed within the last six months.

Compensation and Benefits

This is a full-time position with a salary range of $40.409-$43.169 per hour. We offer a comprehensive benefits package and opportunities for career growth and development.
