**Job Summary:**

We are seeking a highly skilled and compassionate Bilingual Care Coordinator - Community Health Specialist to join our team at Home and Community Care Support Services. As a key member of our community health team, you will work closely with patients, families, caregivers, and healthcare providers to deliver high-quality care and services.

About the Role:

  • You will assess patient needs, develop individualized care plans, and coordinate services to support optimal health outcomes.
  • You will work collaboratively with healthcare teams, including doctors, nurses, and social workers, to ensure seamless care coordination.
  • You will communicate effectively with patients and their families, addressing their concerns and preferences while ensuring safe and effective care.
  • You will provide education and support to patients and caregivers on health conditions, treatment options, and available resources.
  • You will facilitate understanding and cooperation among healthcare team members to maximize patient outcomes.

About You:

  • You hold a degree or diploma in Nursing, Physiotherapy, Occupational Therapy, Social Work, Speech Therapy, or a related field, and are registered with the relevant professional organization.
  • You have 2 years of experience in community-based care, preferably in case management, social work, or a related field.
  • You possess excellent communication, interpersonal, and problem-solving skills, with the ability to work effectively in a team environment.
  • You are proficient in Microsoft Office applications and have basic computer skills.
  • You are fluent in English and French, with the ability to communicate effectively with patients, families, and healthcare teams in both languages.
