Position Overview:

Our Rehabilitation Neuro Unit is a vibrant 40-bed facility specializing in acute medical care, acute stroke/neurology, and rehabilitation for stroke and neurology patients. As a Licensed Practical Nurse (LPN) within our collaborative healthcare team, which includes Occupational Therapists, Physiotherapists, Speech Language Pathologists, Social Workers, Nurse Practitioners, and RN/RPN/PSW care providers, you will exemplify clinical excellence and adhere to the professional standards established by the College of Nurses of Ontario and Southlake Regional Health Centre. Your responsibilities will encompass providing direct patient care, collaborating with fellow healthcare professionals, and ensuring the delivery of safe and high-quality care.

Key Responsibilities:

  • Deliver patient-centered care within the LPN scope of practice
  • Work alongside the healthcare team to provide care for patients with intricate health conditions
  • Participate in the continuous assessment and documentation of patient health
  • Assist in formulating and executing personalized care plans
  • Guide and mentor new staff and students
  • Promote best practices in care delivery
  • Perform additional duties as required

Skills & Competencies:

  • Preferred experience in acute general medicine nursing
  • Skilled in initiating and maintaining IV therapy
  • Strong organizational and prioritization abilities
  • Commitment to ongoing professional development and learning
  • Focus on patient and staff safety
  • Excellent communication and problem-solving skills
  • Experience with diverse medical populations and wound management
  • Familiarity with Electronic Documentation systems
  • Demonstrated leadership and teamwork capabilities

Qualifications:

  • Diploma in Practical Nursing from an accredited institution
  • Active LPN Certificate of Registration from the College of Nurses of Ontario
  • Current Basic Life Support (BLS) certification
  • Completion of a Physical Assessment course is preferred
