Position Overview

The Licensed Practical Nurse (LPN) exemplifies clinical proficiency and adheres to the core competencies as outlined by the College of Nurses of Ontario (CNO), along with the policies and procedures established by Southlake Regional Health Centre. The LPN collaborates effectively as a vital member of the healthcare team, delivering direct patient care in accordance with the three-factor framework defined by the CNO standard, which includes the Client, Nurse, and Environment. These elements significantly influence decision-making regarding care assignments and the necessity for interprofessional collaboration.

Key Responsibilities


• Deliver patient and family-centered care within the LPN scope of practice, utilizing the three-factor framework of the College of Nurses of Ontario.

• Operate in compliance with established hospital policies and procedures, including emergency protocols.

• Ensure practice aligns with entry-to-practice competencies, standards, guidelines, and legislation.

• Communicate effectively with patients, families, and all healthcare team members.

• Collaborate with Registered Nurses (RNs) to provide comprehensive care for patients with complex health conditions.

• Work alongside other interprofessional healthcare team members.

• Provide consultation and clinical support to colleagues as necessary.

• Contribute to the ongoing assessment and documentation of each patient's health status.

• Assist in the development and implementation of the Plan of Care, evaluating its effectiveness regularly.

• Support the orientation and mentorship of new staff and students in clinical settings.

• Serve as a role model, promoting the integration of safe and best practices into daily activities to ensure exemplary care.

• Perform other duties as assigned by the Manager or delegate.

Qualifications and Skills


• Diploma in Practical Nursing from an accredited institution.

• Current RPN Certificate of Registration in good standing with the College of Nurses of Ontario.

• Valid BLS certification required.

• Physical assessment training preferred.

• Recent experience in acute general medicine nursing is advantageous.

• Familiarity with IV therapy initiation and maintenance is preferred.

• Experience in health education is beneficial.

• Ability to work 12-hour shifts, including nights and weekends.

• Knowledge of complex and geriatric medical populations is essential.

• Demonstrated expertise in skin assessment and wound care.

• Strong organizational skills to prioritize and respond to patient needs effectively.

• Commitment to continuous professional development and quality improvement.

• Excellent verbal and written communication skills.

• Strong analytical and problem-solving abilities.

• Dedication to patient and family-centered care and teamwork.
