Medical Case Manager

We are seeking a highly skilled and compassionate Nurse Case Manager to join our team at CB Canada. As a key member of our healthcare support team, you will play a vital role in providing exceptional patient care and support to our clients.
Key Responsibilities- Provide medical support and education to patients and healthcare providers, including disease management and therapy guidance.
- Offer telephone support and maintenance calls to ensure seamless patient care.
- Manage and audit patient databases to ensure accurate and up-to-date information.
- Administer and adhere to case-management protocols, including clinical monitoring, data collection, and patient follow-up.
- Collaborate with healthcare providers, pharmacies, and infusion clinics to coordinate patient care and resolve issues.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Participate in training and development programs to enhance skills and knowledge.
- Bachelor's degree or registered nurse (RN) diploma, with a minimum of 5 years of nursing experience.
- Minimum of 2 years of experience in public or private third-party reimbursement, pharmaceutical industry, or clinical support.
- Strong patient focus, ability to multi-task, and excellent communication skills.
- Ability to work under pressure, with strong problem-solving skills and a customer-centric approach.
- Proficiency in Microsoft Office and other relevant software applications.
