Job Summary

We are seeking a highly skilled and compassionate Nurse Case Manager to join our team at CB Canada. As a key member of our healthcare support team, you will play a vital role in providing exceptional patient care and support to our clients.

Key Responsibilities
  • Provide medical support and education to patients and healthcare providers, including disease management and therapy guidance.
  • Offer telephone support and maintenance calls to ensure seamless patient care.
  • Manage and audit patient databases to ensure accurate and up-to-date information.
  • Administer and adhere to case-management protocols, including clinical monitoring, data collection, and patient follow-up.
  • Collaborate with healthcare providers and pharmaceutical teams to secure funding for patient therapies.
  • Participate in training sessions and workshops to enhance knowledge and skills.
  • Establish and maintain partnerships with healthcare providers, pharmacies, and infusion clinics.
  • Communicate effectively with patients, providers, and internal teams to ensure seamless care coordination.
Requirements
  • Bachelor's degree or registered nurse (RN) diploma, with a minimum of 5 years of nursing experience.
  • Minimum of 2 years of experience in public or private third-party reimbursement, pharmaceutical industry, or clinical support.
  • Strong patient focus, ability to multi-task, and excellent communication skills.
  • Proficiency in Microsoft Office and ability to work autonomously.
  • Available for travel at least 15% per year.
