Job Title: Rehabilitation Worker

March of Dimes Canada is seeking a highly skilled and compassionate Rehabilitation Worker to join our Community Support Services team in Newmarket, Ontario.

About the Role:

We are looking for a dedicated and experienced professional to work with individuals with brain injuries, providing essential services that enable them to thrive in their communities.

Key Responsibilities:
  • Implement programs designed to enhance the efficiency and safe delivery of Community Support Services to clients with Brain Injury with the support of the Rehab Service Coordinator (RSC).
  • Work collaboratively with the RSC in completing all daily report writing, incident reports, Individual Rehabilitation Plan, and quarterly reporting.
  • Act as a resource to staff and families in the provision of long-term support and service coordination for complex or catastrophic cases to enable adults with Brain Injuries to remain in community.
  • Provide cognitive and compensatory strategies that support functional goal setting.
Requirements:
  • 2 – 5 years experience in the Social Services/Health Sector, coordinating services for individuals with Brain Injury is an asset.
  • Concrete knowledge of the effects of an ABI is an asset; training will be provided.
  • Community-based experience working with individuals with a Brain Injury, physical disabilities or special needs.
  • General understanding of computer hardware and software.
