Job Responsibilities:- Hold a Master's degree in Social Work from an accredited university.
- Possess prior experience working with patients facing cardiovascular conditions; this is preferred.
- Experience in Chronic Disease Management and Cardiac Rehabilitation is highly advantageous.
- Maintain active membership in the Ontario College of Social Workers and Social Service Workers.
