**About the Role**

We are seeking a highly skilled and compassionate Brain Injury Rehabilitation Worker to join our Community Support Services team at March of Dimes Canada. As a key member of our team, you will play a vital role in providing essential services to individuals with brain injuries, enabling them to live and thrive in their communities.

Key Responsibilities:

  • Implement programs designed to enhance the efficiency and safe delivery of Community Support Services to clients with Brain Injury, working collaboratively with the Rehab Service Coordinator (RSC).
  • Conduct daily report writing, incident reports, Individual Rehabilitation Plan, and quarterly reporting, ensuring accurate and timely documentation.
  • Provide cognitive and compensatory strategies to support functional goal setting, empowering individuals with brain injuries to achieve their full potential.
  • Collaborate with staff and families to provide long-term support and service coordination for complex or catastrophic cases, promoting independence and community integration.

Requirements:

  • 2-5 years of experience in the Social Services/Health Sector, coordinating services for individuals with Brain Injury (asset).
  • Knowledge of the effects of an Acquired Brain Injury (ABI) is an asset; training will be provided.
  • Community-based experience working with individuals with brain injuries, physical disabilities, or special needs.
  • General understanding of computer hardware and software.
