About the Role
We are seeking a skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing and determining patient care needs and eligibility, providing access and referrals to community services, and engaging with patients, caregivers, and other healthcare practitioners.
Key Responsibilities
- Collaborate with patients and their families to assess care needs, determine eligibility for services, and develop individual care and service plans
- Link patients with service providers and coordinate care plan delivery
- Establish a helping relationship with patients and their families, balancing their needs and choices with available resources
- Work effectively in a multidisciplinary environment, prioritizing tasks and managing time to meet deadlines
Requirements
- Membership in good standing with the applicable regulatory body (e.g., College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.)
- Recent experience in community health or a related field
- Knowledge of the healthcare delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
- Effective time management, prioritization, and organizational skills
