Position Overview

Under the general oversight of the Team Manager and while demonstrating a commitment to the mission, vision, and values of WRHA Community Health Services, the Licensed Practical Nurse (LPN) is tasked with delivering professional practical nursing services to designated clients. This includes independent nursing care, administration of prescribed medical treatments, collaboration with the healthcare team, and participation in the coordination of client care and discharge processes. Services may be provided in various settings such as homes, schools, workplaces, or at a home care nursing clinic.

Experience

  • Preferred: 2 years of recent related experience.
  • Required: Satisfactory employment record.

Education (Degree/Diploma/Certificate)

  • Required: Graduate of an accredited Licensed Practical Nursing Education program.
  • Preferred: Additional training in Wound Care & Diabetes.

Certification/Licensure/Registration

  • Responsible for maintaining and providing proof of active registration with the College of Licensed Practical Nurses of Manitoba (CLPNM).
  • CPR Training requirements must align with Employer policy.
  • Valid Manitoba Class 5 driver's license is required.

Qualifications and Skills

  • Knowledge of: Standards of Practice and Code of Ethics for Licensed Practical Nurses as set by the College of Licensed Practical Nurses of Manitoba.
  • Understanding of the scope of practice as outlined in the Licensed Practical Nurses Act.
  • Familiarity with regional, facility, and unit policies, procedures, protocols, and guidelines.
  • Comprehensive nursing care knowledge related to community nursing.
  • Awareness of the Personal Health Information Act (PHIA), Protection of Persons in Care Act, Mental Health Act, Workplace Hazardous Material Information System (WHMIS), and Principles of Routine Practices (Universal Precautions).
  • Roles and responsibilities of healthcare team members.
  • Principles of delegation and assignment, as well as adult learning and teaching strategies.
  • Demonstrated effective oral and written communication skills.
  • Ability to delegate and assign care appropriately while monitoring outcomes.

Physical Requirements

  • Moderate to heavy physical effort is required.
  • Must be physically capable of providing services in diverse community settings under varying weather and environmental conditions.
  • Use of a reliable motor vehicle suitable for all environmental conditions is necessary.
  • Subject to immunization and tuberculin testing as per WRHA policy.
