Position Overview:

The role of the Program Services Caseworker is essential in assisting clients and ensuring the smooth operation of the residential facility. This position involves:

  • Client admissions and intake processing
  • Case triaging and primary support casework
  • Referral and clerical responsibilities

The objective is to provide clients with motivation and access to comprehensive support services that facilitate the achievement and maintenance of stable housing. This position plays a crucial role in the prevention and eradication of youth homelessness while addressing the essential needs of vulnerable groups in the community.

Key Responsibilities:

Case Management and Facility Operations:

  • Manage an active caseload effectively.
  • Oversee daily shift responsibilities and reporting obligations.
  • Conduct intake assessments, develop service plans, and deliver case management and clinical interventions for assigned clients.
  • Maintain ongoing responsibility for housing stability planning and related casework.
  • Document activities and incidents accurately during shifts using Logbook and/or Incident Reports.
  • Engage emergency services when necessary.
  • Uphold confidentiality regarding client and organizational records.
  • Establish a network and make referrals to community services and resources, including those offered by other departments.
  • Utilize the HIFIS system for all aspects of client case management.
  • Participate in maintaining cleanliness and upkeep of facilities when necessary.
  • Adhere to outlined responsibilities as per shift sheets.

Administrative Duties:

  • Report monthly performance statistics related to casework.
  • Ensure daily updates and documentation in the HIFIS system.
  • Complete daily logs and bed board charts.
  • Handle all relevant documentation, including intake and discharge forms, client notes, shift logs, and incident reports.
  • Maintain a personal work account for strategic purposes, including email and attendance systems.

Working Conditions:

  • This is a permanent full-time position.
  • Shift schedules average 84 hours biweekly, approximately 42 hours weekly.
  • Work is conducted onsite.
  • Minimal travel is required.

Qualifications and Education:

  • Completion of a college diploma or equivalent post-secondary education.
  • A Bachelor of Social Work is preferred.
  • A valid Ontario Class 'G' Driver's License is required.

Experience and Knowledge:

  • Experience in working with homelessness and mental health issues.
  • Functional knowledge related to homelessness and mental health.
  • Commitment to anti-racism, anti-oppression, and equity principles.
  • Familiarity with community resources.

Skills and Capabilities:

  • Strong oral and written communication skills.
  • Ability to work independently and collaboratively.
  • Effective interpersonal and time management skills.
  • Willingness to undergo necessary background checks.
  • Commitment to understanding and supporting the mission of The Salvation Army.
