Job Title: Full Time Program Services Caseworker

We are seeking a dedicated and compassionate Full Time Program Services Caseworker to join our team at The Salvation Army. As a key member of our residential facility, you will play a vital role in supporting clients and ensuring the smooth operation of our day-to-day activities.

Key Responsibilities:
  • Maintain an active caseload and provide primary support casework to clients.
  • Responsible for client admissions and intake processing, case triaging, and referral duties.
  • Complete intake and assessments, creating service plans, and providing case management services and clinical interventions for an assigned caseload.
  • Record activities and incidents occurring during the shift in a factual and concise manner using Logbook and/or Incident Occurrence Reports.
  • Develop a network and make referrals to existing services and resources in the community.
Administrative Responsibilities:
  • Responsible for reporting monthly PSCW performance statistics.
  • Ensure daily updates, submissions, assessments, incidents, and housing plans are completed in HIFIS.
  • Complete daily front-line worker responsibility logs and bed board charts.
  • Perform all relevant documentation related to PSCW, including intake/discharge forms, client HIFIS notes, shift logs, incident reports, and client forms.
Requirements:
  • College diploma or equivalent post-secondary education.
  • Valid Ontario Class "G" Driver's License and a current copy of a Driver's abstract.
  • Experience working with homelessness, mental health, and related services.
  • Thorough knowledge and understanding of anti-racism, anti-oppression, and equity principles.
