Caregiver - Home Support Services

The Personal Support Worker provides personalized care and support services to clients and families within the framework of VON policies, procedures, and standards. This role involves collaborating with healthcare professionals to meet the needs of clients and promote their independence and well-being.
Key Responsibilities- Conduct thorough assessments of clients' homes, personal situations, and care plans to identify areas for improvement and ensure their needs are met.
- Develop and implement personalized care plans under the guidance of the Personal Support Program Manager, taking into account the client's and family's needs, safety, and liability.
- Provide personal care services, including light housekeeping, meal preparation, and assistance with daily living activities, as directed by the Personal Support Program Manager or immediate supervisor.
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care and support services.
- Monitor and maintain household safety, report any changes or concerns to the immediate supervisor or Personal Support Program Manager.
- Perform other duties as assigned, including respite care, child care, and household management.
- High school diploma or equivalent.
- Completion of a Personal Support Worker program or equivalent.
- Current Standard First Aid Certificate or CPR level C Certificate, if applicable.
- Experience working with elderly or ill individuals or children, preferably in a home care setting.
- Strong communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
- Ability to maintain confidentiality and adhere to strict confidentiality protocols.
- Valid driver's license and access to a personal vehicle for work-related purposes.
This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in this role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
