Job Summary

The Personal Support Worker provides personalized support and homemaking services to clients and families within the framework of VON policies, procedures, and standards. The PSW collaborates with members of the healthcare team to meet the needs of clients and attain an optimal level of independence and well-being.

Key Responsibilities
  • Conduct thorough assessments of clients' homes, personal situations, and care plans to identify areas for improvement and ensure client needs are met.
  • Develop and implement personalized care plans under the guidance of the Personal Support Program Manager, ensuring safety, liability, and financial considerations.
  • Provide personal support tasks, including light cleaning, meal preparation, and household safety monitoring, tailored to clients' needs.
  • Assist clients with daily activities, such as walking, climbing stairs, and personal hygiene, as directed by the Personal Support Program Manager or immediate supervisor.
  • Provide respite care and support to clients' families, including childcare, to allow for family relief.
  • Communicate effectively with clients, families, and healthcare teams to ensure seamless care coordination.
  • Maintain accurate records and complete daily call checks to ensure efficient service delivery.
Requirements
  • High school diploma or equivalent.
  • Certificate in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide.
  • Current Standard First Aid Certificate or CPR level C Certificate, if applicable.
  • Experience working with elderly or ill individuals or children, with a focus on household management.
  • Strong communication and assessment skills, with the ability to work effectively in a multidisciplinary team.
Working Conditions

This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying, bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in this role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
