Key Responsibilities:

Program Philosophy

  • Deliver culturally appropriate services as necessary.
  • Adhere to the principles of Harm Reduction and Housing First methodologies.
  • Possess a thorough understanding of the Residential Tenancy Act.
  • Comply with established program policies and procedures.

Administrative Duties

  • Implement practices aligned with the Standards of Accreditation (CAC).
  • Conduct monthly evidence-based assessments with program participants and ensure accurate data entry (HMIS and CIVI, etc.).
  • Develop, execute, and revise client-centered service plans, safety strategies, wellness agreements, and treatment outlines.
  • Complete daily administrative responsibilities during shifts.

Client Support

  • Provide case management services utilizing an Intensive Case Management approach for individuals transitioning from homelessness.
  • Assist program participants facing acute, severe, and persistent mental health and substance use challenges.
  • Connect clients with suitable community services and facilitate necessary referrals.
  • Educate and support participants in daily living skills within their residences and community settings (e.g., cooking, cleaning, financial management, grocery shopping, budgeting, laundry, etc.).
  • Identify potential eviction risks and collaborate with participants to mitigate these risks, including supporting monthly room inspections.
  • Offer transportation assistance to external appointments and aid with daily living activities.
  • Advocate for the rights and needs of program participants.
  • Assist participants in navigating various systems, including justice, financial, and healthcare.
  • Support participants in obtaining and maintaining income assistance.
  • Help program participants with rent payments and create rental arrears repayment plans as necessary.
  • Provide medication assistance as required.
  • Exhibit strong crisis intervention and de-escalation skills.
  • Maintain openness to a broad scope of practice with a commitment to innovation and adaptability.

Team Collaboration

  • Exhibit exceptional written and verbal communication skills, including active listening.
  • Engage in team and treatment planning discussions.
  • Demonstrate initiative and the ability to function effectively as part of a team with minimal supervision, while supporting colleagues as needed.
  • Maintain a high level of professionalism and collaborate with partner organizations.
  • Encourage and provide constructive feedback to foster a positive workplace atmosphere, strong team cohesion, and outstanding program delivery.
  • Contribute positively to team dynamics.

Program Responsibilities

  • Comply with all health and safety regulations within the facility.
  • Perform daily cleaning tasks and assist with preparation and cleanup for participant activities as required (meals, recreation, etc.).
  • Facilitate monthly recreational group activities.

Qualifications:

  • A Bachelor's degree in a relevant field is preferred; a diploma with relevant experience will also be considered.
  • Experience in the homelessness sector, along with a strong understanding of community resources, is essential. Practicum experience will be acknowledged.
  • Knowledgeable and comfortable working within harm reduction, housing first, strength-based, natural and logical consequences, and trauma-informed care approaches, as well as an understanding of anti-oppressive practices.
  • A solid foundation in addictions and mental health, including knowledge of schizophrenia, personality disorders, PTSD, and trauma.
  • Skills and abilities to work with complex individuals facing challenges related to addictions, mental health, isolation, homelessness, and poverty.
  • Ability to work independently and confidently in conflict situations or with at-risk populations.
  • Ability to engage with a diverse range of individuals, demonstrating a commitment to continuous learning and providing appropriate person-centered support for LGBTQ2IA+, Indigenous peoples, and individuals from culturally diverse backgrounds.
  • Exceptional communication, organizational, advocacy, teaching, and rapport-building abilities.
  • Capacity to adapt and effectively establish relationships with hard-to-engage individuals while maintaining appropriate boundaries and ethical standards.
  • A professional and positive demeanor, with the ability to represent the organization effectively.
  • Experience with management of alcohol programming and opioid replacement therapy (Suboxone and Methadone) is considered an asset.
  • First Aid certification (Standard/Intermediate or Emergency/Basic) - Level C CPR within 30 days of hire, with certification valid for a minimum of 6 months upon hire.
  • A valid Class 5 non-GDL driver's license, a clean driving record, and a personal vehicle available for work purposes with $2 million in liability coverage are required.
  • A satisfactory vulnerable sector criminal record clearance is mandatory.
  • Familiarity with the United Nations Declaration on the Rights of Indigenous Peoples (UNDRIP) and the Truth and Reconciliation Commission's (TRC) 94 Calls to Action is an asset.

Work Environment:

  • The Prelude operates as a 24/7 residential program; the majority of this role is on-site, with occasional mandatory and/or voluntary training that may occur offsite.
  • This position requires 1 FTE (37.5 hours/week); typical hours are 7 am – 3 pm, Monday through Friday.
  • Involves continuous extended periods of sitting, standing, and walking.
  • Frequent climbing and carrying objects up or down stairwells.
  • Occasional driving as part of work responsibilities.
  • Occasional physical activity; requires the ability to bend, reach above shoulder height, kneel, crawl, and carry objects (up to and over 20 lbs.) throughout the shift, including lifting or transferring clients who require assistance.
  • Occasional outdoor work in various weather conditions.

Program Overview: The Prelude is a low-barrier Place-Based Supportive Housing (PBSH) initiative that offers safe and stable housing to adults with a history of homelessness and significant vulnerabilities due to concurrent health disorders. Our philosophy asserts that everyone is entitled to suitable housing and the necessary supports to maintain that housing. We deliver our services through an Intensive Case Management model, adhering to the principles of harm reduction, housing first, and strength-based practice. Our residents range from 18 to over 70 years old, facing tri-morbidity, polysubstance use, and cognitive barriers to independent living (such as brain injury, dementia, etc.).

Working Conditions: This position requires frontline staff to be exposed to emotionally challenging situations, including medical emergencies, threats or actual serious injury, violence, abuse, and death; clients may experience fear, hopelessness, rage, self-harm, and conflict. It is understood that staff will work in high-stress environments, which may include emergency responses where resilience, stress tolerance, and the ability to maintain composure and focus are essential. Exposure to bodily fluids may also occur.
