The Alex is seeking a dedicated individual to join our team as a Case Manager within our supportive housing program. This position plays a crucial role in providing comprehensive support to individuals facing homelessness and related challenges.
Key Responsibilities:
Program Philosophy:
- Deliver culturally aware services tailored to individual needs.
- Adhere to the principles of Harm Reduction and Housing First.
- Familiarize yourself with the Residential Tenancy Act.
- Comply with established program policies and procedures.
Administrative Duties:
- Implement the Standards of Accreditation (CAC) in daily operations.
- Conduct monthly evidence-based assessments and manage necessary data entry.
- Develop, implement, and revise client-centered service plans and treatment strategies.
- Fulfill daily administrative responsibilities.
Client Support:
- Provide case management services using an Intensive Case Management approach for individuals transitioning out of homelessness.
- Assist program participants dealing with severe mental health and substance use challenges.
- Facilitate connections to community resources and support referral processes.
- Guide participants in developing essential daily living skills.
- Identify potential eviction risks and collaborate with participants to mitigate these risks.
- Offer transportation assistance for external appointments and daily living activities.
- Advocate for participant rights and support systems navigation.
- Assist with income support applications and rental payment plans.
- Support medication management as necessary.
- Exhibit strong crisis intervention and de-escalation capabilities.
- Maintain a flexible and innovative approach to service delivery.
Team Collaboration:
- Demonstrate effective communication skills, including active listening.
- Engage in team meetings and collaborative treatment planning.
- Work independently while supporting team members as needed.
- Foster a professional atmosphere and collaborate with partner organizations.
- Encourage constructive feedback to enhance workplace culture and program effectiveness.
Program Responsibilities:
- Ensure compliance with health and safety regulations.
- Assist with cleaning and preparation for participant activities.
- Lead monthly recreational group sessions.
Qualifications:
- A Bachelor's degree in a relevant field is preferred; a diploma with relevant experience is acceptable.
- Experience in the homelessness sector and familiarity with community resources is essential.
- Knowledge of harm reduction, housing first principles, and trauma-informed care is required.
- Strong understanding of mental health and addiction issues.
- Ability to work effectively with diverse populations and maintain professional boundaries.
- Exceptional communication, organizational, and advocacy skills.
- Ability to adapt and build rapport with individuals facing challenges.
- Possession of a valid Class 5 non-GDL driver's license and a satisfactory criminal record clearance.
Working Environment:
- This role requires on-site presence with occasional off-site training.
- Full-time position (37.5 hours/week) with a schedule from 7 am to 3 pm, Monday to Friday.
- Physical demands include prolonged periods of sitting, standing, and walking, as well as occasional lifting and driving.
