Licensed Practical Nurse (LPN)

We are currently looking for an experienced healthcare professional to join our team and assist in broadening our community outreach and impact.

Position Overview: The Community Outreach (LPN) Nurse will be instrumental in establishing and nurturing connections within the community, generating new client leads, and coordinating in-home client evaluations and care plan development.
This role requires a proactive individual with outstanding communication abilities, a passion for senior care, and the capacity to work autonomously while collaborating with our internal team.

Key Responsibilities:

Community Relationship Development:

  • Foster and sustain robust relationships with community stakeholders, including healthcare providers, social workers, and senior service organizations.
  • Serve as a liaison between the organization and the community, addressing inquiries and concerns with professionalism and empathy.
  • Participate in community events, health fairs, and networking opportunities to promote our services and cultivate relationships.

Client Evaluations:

  • Perform comprehensive client evaluations to assess their care needs and eligibility for in-home services.
  • Collaborate with our Client Services Manager to create personalized care plans based on evaluation results.
  • Communicate clearly and compassionately with clients and their families regarding care options and services.

Qualifications:

  • LPN (Licensed Practical Nurse) certification in British Columbia.
  • At least 2 years of experience in senior care, home health, or a related field.
  • Experience in community outreach is a plus.
  • Strong interpersonal and communication skills, with the ability to build rapport with diverse populations.
  • Exceptional organizational skills and attention to detail.
  • Self-motivated: Ability to work independently and as part of a team in a dynamic environment.
