Position Overview: The Community Health Nurse will be instrumental in fostering connections within the community, generating new client opportunities, and assisting with in-home client evaluations and care plan development.

Key Responsibilities:

  • Building Community Relationships:
    • Establish and maintain strong connections with community partners, including healthcare professionals and senior service organizations.
    • Inform the community about the advantages of in-home senior care and the services provided by Senior Helpers.
    • Serve as a liaison between the organization and the community, addressing inquiries and concerns with professionalism and empathy.
  • Client Acquisition:
    • Develop and execute strategies to identify and attract potential clients for in-home care services.
    • Collaborate with local healthcare facilities and senior living communities to create referral networks and drive new business.
    • Participate in community events and networking opportunities to promote our services and build connections.
  • Client Evaluations:
    • Accompany the Client Service Manager to client residences as needed to conduct comprehensive assessments of care needs.
    • Work with the Client Services Manager to create tailored care plans based on assessment results.
    • Communicate clearly and compassionately with clients and their families regarding care options and services.
  • Administrative Duties:
    • Maintain accurate records of outreach activities, client interactions, and assessments.
    • Prepare reports on lead generation, community engagement, and client assessment outcomes.
    • Assist in developing marketing materials and strategies to enhance outreach efforts.

Qualifications:

  • LPN or RN license in British Columbia.
  • At least 2 years of experience in senior care, home health, or a related field.
  • Experience in community outreach, sales, or business development is preferred.
  • Strong interpersonal and communication skills, with the ability to connect with diverse populations.
  • Exceptional organizational skills and attention to detail.
  • Self-motivated: Ability to work independently and collaboratively in a dynamic environment.
