Community Outreach Licensed Practical Nurse

We are currently in search of a skilled medical professional to join our team and assist in broadening our community outreach and influence.
Position Overview: The Community Outreach Nurse (LPN) will be instrumental in fostering and maintaining relationships within the community, generating new client referrals, and coordinating in-home client evaluations and care plan development.
This role requires a proactive individual with outstanding communication abilities, a genuine passion for senior care, and the capability to work autonomously while collaborating with our internal team.
Key Responsibilities:
Building Community Relationships:
- Develop and sustain strong connections with community stakeholders, including healthcare providers, social workers, and senior service organizations.
- Serve as a representative of the company within the community, addressing inquiries and concerns with professionalism and empathy.
- Participate in community events, health fairs, and networking opportunities to promote our services and establish connections.
Client Evaluations:
- Perform comprehensive client evaluations to assess their care needs and determine suitability for in-home services.
- Collaborate with our Client Services Manager to create personalized care plans based on evaluation results.
- Communicate clearly and compassionately with clients and their families regarding care options and services.
Qualifications:
- LPN (Licensed Practical Nurse) certification in British Columbia.
- At least 2 years of experience in senior care, home health, or a related field.
- Experience in community outreach is a plus.
- Strong interpersonal and communication skills, with the ability to connect with diverse populations.
- Exceptional organizational skills and attention to detail.
- Self-motivated: Ability to work independently and as part of a team in a dynamic environment.
