  • Collaborate with healthcare professionals, families, and community resources to facilitate effective discharge planning and post-discharge support.
  • Advocate for client rights and needs, connecting them with appropriate services and resources within the community.
  • Contribute to a culture of continuous learning and improvement by participating in professional development opportunities and sharing best practices with colleagues.

The ideal candidate possesses a Master of Social Work degree, registration with relevant professional bodies, and a minimum of two years of clinical experience in a healthcare setting.

A strong understanding of human behavior, including grief and loss, caregiving dynamics, and crisis intervention strategies is essential. Excellent communication, interpersonal, and problem-solving skills are highly valued. Fluency in multiple languages is an asset.
