About the Opportunity

Nova Scotia Health is seeking a skilled Health Care Social Worker to join our team in the Western Zone, Yarmouth Regional Hospital. As a member of our interprofessional team, you will play a vital role in delivering Primary Health Care and chronic disease management services, with a focus on chronic pain management.

The successful candidate will possess a Master's degree in social work from a recognized university and current registration with the Nova Scotia College of Social Workers. They will have a minimum of 2 years of post-MSW clinical experience and a proven ability to use a client-/family-centered approach. Experience working in Primary Care and/or community-based settings is an asset.

The ideal candidate will be able to demonstrate excellent communication skills, the ability to work collaboratively with an interdisciplinary team, and a commitment to diversity, equity, and inclusion. They will also have a valid Nova Scotia Driver's license and access to a reliable vehicle.

This is a permanent full-time position, with 75 hours bi-weekly. The successful candidate will be required to work days, evenings, and weekends, and may be required to travel to various sites.

The compensation for this position is $49.04 hourly, and successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.

About You
  • Master's degree in social work from a recognized university
  • Current registration with the Nova Scotia College of Social Workers
  • 2 years post MSW related clinical experience or relevant work experience required
  • Previous experience working in Primary Care and/or community-based setting preferred
  • Demonstrated ability to use a client-/family-centered approach
  • Psychotherapy skills (i.e. ACT and CBT)
  • Familiarity with various evidence-based treatment approaches including Motivational Interviewing and behaviour change counselling
  • Demonstrated experience with group facilitation
  • Ability to complete biopsychosocial assessments
  • Excellent communication skills
  • Demonstrated ability to assist patients and families with navigation of local community resources and agencies
  • Ability to demonstrate effective time management and disciplined self-organization
  • Demonstrated comfort and skills working collaboratively with an interdisciplinary team and community partners
