North York General Hospital is seeking a highly skilled and experienced Dietitian to join our Ambulatory Care team. As a key member of our inter-professional healthcare team, you will provide safe, effective, and ethical care for patients and their families.
Key Responsibilities:- Provide client-centred assessments of emotional, physical, cognitive, and social needs and capabilities of patients and their families.
- Develop and implement personalized nutrition care plans to meet the unique needs of patients with diabetes.
- Collaborate with the inter-professional team to provide comprehensive care and support to patients and their families.
- Participate in medical team meetings and inter-professional rounds to ensure seamless communication and coordination of care.
- Stay up-to-date with the latest evidence-based practices and guidelines in diabetes management and nutrition care.
- Successful completion of a Foods & Nutrition program accredited by the Partnership for Dietetic Education and Practice (PDEP) or the Accreditation Council for Education in Nutrition and Dietetics (ACEND).
- Registered member, in good standing, of the College of Dietitians of Ontario.
- Minimum 2-5 years recent outpatient clinical experience and expertise in diabetes management and education.
- Certified Diabetes Educator (CDE) preferred.
