Ambulatory Care Dietitian with Diabetes Expertise

We are seeking a skilled Ambulatory Care Dietitian to join our team at North York General Hospital. As a key member of our inter-professional healthcare team, you will provide safe and effective care for patients with diabetes.
The successful candidate will have expertise in managing complex diabetes cases and be able to work independently as well as collaboratively within our team.
You will be responsible for providing client-centered assessments, developing and implementing nutritional care plans, and collaborating with the inter-professional team to achieve optimal outcomes.

Responsibilities- Provide client-centered assessments and develop personalized nutritional care plans
- Collaborate with the inter-professional team to achieve optimal outcomes
- Participate in medical team meetings and inter-professional rounds
- Counsel individuals and groups in normal and therapeutic nutrition
- Maintain excellent knowledge of hospital and community resources
- Degree in human nutrition and dietetics from a recognized university program
- Accredited dietetic internship or equivalent supervised practical training
- Member in good standing with the College of Dietitians of Ontario
- Certified Diabetes Educator
- 1-year recent clinical experience and expertise in diabetes management and education
We offer a competitive total compensation package, including benefits, pension, and vacation time. This is a Full-Time Reduced Hour position with 0.8 FTE, 8 hour shifts, day shift, evening shift, including weekends.
An estimated salary for this position is $83,000 - $110,000 per year, based on location and experience.
