Job Opportunity:

North York General Hospital is seeking a skilled Registered Dietitian to join our dynamic team in the Diabetes Clinic.

Job Summary:

The successful candidate will provide safe, effective, and ethical care for patients and their families, focusing on nutritional care in accordance with established standards. This role involves working with the inter-professional healthcare team to provide client-centred assessments, self-management directed care, and leadership in developing patient care initiatives.

Key Responsibilities:

  • Provide client-centred assessments of emotional, physical, cognitive, and social needs and capabilities of patients and their families.
  • Provide self-management directed care to patients with diabetes and their families/caregivers.
  • Take on a leadership role in developing and participating in centre-specific and corporate patient care initiatives related to diabetes.
  • Prepare dietetic nutritional plans of care that meet patient needs in collaboration with the inter-professional teams.
  • Chart written assessments and progress notes on patient health records; monitor, reassess, and evaluate for optimal outcomes.
  • Recommend and interpret therapeutic diet prescriptions/nutrition support orders by utilizing best practice guidelines.
  • Advocate for the patient/family and community to ensure patients' rights are recognized and appropriate services are created/provided.
  • Formulate and facilitate the discharge plan with patient, family, and healthcare team utilizing appropriate community resources.

Requirements:

  • Successful completion of a Foods & Nutrition program accredited by the Partnership for Dietetic Education and Practice or the Accreditation Council for Education in Nutrition and Dietetics.
  • Registered member, in good standing, of the College of Dietitians of Ontario.
  • Certified Diabetes Educator preferred.
  • Minimum 2-5 years recent outpatient clinical experience and expertise in diabetes management and education.
  • Current teaching experience and counseling background.
  • Current experience with electronic documentation and computer proficiency.
  • Current knowledge of normal and therapeutic nutrition, enteral feeds, and TPN.

North York General Hospital offers a highly competitive total compensation package, including benefits, pension, and vacation. This is a part-time temporary position with 8-hour days and evening shifts, including weekends in the Diabetes Clinic.
