Job Title: Dietitian, Ambulatory Care

North York General Hospital (NYGH) is seeking a highly skilled and experienced Dietitian to join our Ambulatory Care team. As a key member of our inter-professional healthcare team, you will provide safe, effective, and ethical care for patients and their families.

Job Summary:

The successful candidate will be responsible for providing nutritional care for patients in accordance with the standards of dietetic practice established by the College of Dietitians of Ontario, Dietitians of Canada, and by the hospitals' Clinical Nutrition Services. You will work closely with the inter-professional healthcare team to develop and implement individualized nutrition plans, provide education and counseling to patients and their families, and participate in quality improvement initiatives.

Key Responsibilities:
  • Provide client-centred assessments of emotional, physical, cognitive, and social needs and capabilities of patients and their families
  • Develop and implement individualized nutrition plans that meet patient needs in collaboration with the inter-professional teams
  • Provide education and counseling to patients and their families on normal and therapeutic nutrition, enteral feeds, and TPN
  • Participate in medical team meetings and inter-professional rounds, and act as a liaison and resource person for all staff
  • Maintain excellent knowledge of and close liaison with hospital and community resources
Requirements:
  • Successful completion of a Foods & Nutrition program accredited by the Partnership for Dietetic Education and Practice (PDEP) or the Accreditation Council for Education in Nutrition and Dietetics (ACEND)
  • Registered member, in good standing, of the College of Dietitians of Ontario
  • Minimum 2-5 years recent outpatient clinical experience and expertise in diabetes management and education with a focus in specialized and complex diabetes care
  • Certified Diabetes Educator preferred
