Role Summary
The Clinical Nutrition Specialist, Ambulatory Care will provide safe, effective, and ethical care to patients and their families. This includes providing nutritional care in accordance with the standards of dietetic practice established by the College of Dietitians of Ontario, Dietitians of Canada, and the hospital's Clinical Nutrition Services.
Key Responsibilities
- Provide client-centered assessments of emotional, physical, cognitive, and social needs and capabilities of patients as well as their families.
- Develop and implement patient-centered care plans that meet patient needs in collaboration with the inter-professional teams.
- Provide education and counseling to patients and families on normal and therapeutic nutrition.
- Participate in medical team meetings and inter-professional rounds, and act as a liaison and resource person for all staff.
- Maintain excellent knowledge of hospital and community resources.
- Support the education program for dietetic students by precepting clinical rotations and evaluating them according to the ICDEP competencies.
Requirements
- Successful completion of a 4-year degree in human nutrition and dietetics from a university program accredited by the Partnership for Dietetic Education and Practice (PDEP) or recognized equivalent.
- Successful completion of an accredited dietetic internship or equivalent supervised practical training.
- Member in good standing with the College of Dietitians of Ontario.
- Current teaching experience and counseling background.
- Current experience with electronic documentation along with computer proficiency.

This is a Full-Time Reduced Hour position with 0.8 FTE, 8 hour shifts, day shift, evening shift, including weekends in the Outpatient & Community Services site.
