Position Overview: The Community Health Nurse will be instrumental in fostering and developing connections within the community, generating new client opportunities, and assisting with in-home client evaluations and care planning.

Key Responsibilities:

  • Community Engagement: Build and sustain robust relationships with community partners, including healthcare providers, social service professionals, and senior care organizations.
  • Client Outreach: Create and execute strategies to identify and attract potential clients for in-home care services.
  • Client Evaluations: Collaborate with the Client Service Manager to conduct thorough assessments of client needs and suitability for in-home services.

Qualifications:

  • Current LPN or RN license in British Columbia.
  • At least 2 years of experience in senior care or a related field.
  • Experience in community outreach or business development is preferred.
  • Excellent communication and interpersonal skills.
  • Strong organizational abilities and attention to detail.
