Program Focus: Assisted Living for High-Risk Seniors

Bi-Weekly Hours: 30

Work Hours: Days, Evenings, Weekends

On-Call Duties: Yes

Role Summary:

The Home Care Support Specialist will play a crucial role in delivering companionship, personal assistance, and health maintenance services to clients within their residences or the broader community, adhering to VON's guiding principles, policies, standards, and applicable regulations. In this capacity, you will collaborate with healthcare professionals and personal support teams to ensure clients achieve an optimal level of independence and well-being.

Primary Responsibilities:

  • Assess and monitor the client's or family's circumstances to identify any necessary adjustments to the personal support care plan.
  • Assist in the formulation of the personal support care plan under the supervision of the Personal Support Program Manager.
  • Implement modifications to the care plan as needed for safety and financial considerations.
  • Execute personal support tasks tailored to the specific needs of the client and their family.
  • Deliver personal care as instructed by the Personal Support Program Manager or immediate supervisor.
  • Provide respite care and support to the families of clients as necessary.
  • Organize daily activities and prioritize personal support and related tasks.
  • Evaluate the client's communication patterns and interpersonal skills.
  • Engage with relevant members of the healthcare team regarding the management of the client's requirements.
  • Adhere to Occupational Health and Safety regulations and VON policies.
  • Perform additional duties as assigned.

Qualifications and Experience:

  • Completion of high school education.
  • Certification as a Community Care Assistant is mandatory.
  • Completion of a Home Support Worker, Personal Support Worker program, or Health Care Aide certification.
  • Current Standard First Aid Certificate or CPR level C Certificate.

  • Familiarity with community resources.
  • Ability to work effectively both independently and as part of a team.
  • Flexibility to adapt to various environments.
  • Commitment to maintaining confidentiality.

Additional Requirements:

  • Access to a personal vehicle and possession of a valid driver's license and vehicle insurance.
  • Successful completion of a Criminal Records Check is required.
  • Availability for flexible working hours.
  • Personal Protective Equipment (PPE) will be necessary.

Work Environment and Physical Demands: Fast-paced setting; attention to detail is crucial; ability to lift and carry using proper techniques; capable of walking, sitting, standing, and climbing stairs; proficient in grip and fine motor skills.
