Social Worker II - Concurrent Disorders

We are seeking a highly motivated and experienced Clinical Social Worker to join our dedicated team. In this role, you will provide comprehensive biopsychosocial assessments, consultations, and evidence-based treatment to individuals struggling with complex concurrent substance use, mental health, and physical health disorders.

Responsibilities:
  • Collaborate effectively with a multidisciplinary team of healthcare professionals, including psychiatrists, nurses, psychologists, and community partners.
  • Conduct thorough assessments to understand clients' unique needs and develop individualized treatment plans.
  • Provide individual, group, family, and couples therapy sessions utilizing evidence-informed therapeutic approaches such as motivational interviewing, cognitive behavioral therapy (CBT), and dialectical behavior therapy (DBT).
  • Advocate for clients and their families to ensure access to appropriate resources and support services.
  • Respond effectively to crisis situations, including suicidal ideation and substance use emergencies.
  • Engage with community partners to facilitate a continuum of care and support client integration into the community.
  • Document clinical interventions accurately and comprehensively in accordance with hospital and regulatory standards.
  • Participate in supervision, training, and educational activities for social work students and colleagues.
Qualifications:
  • Master's degree in Social Work (MSW) from an accredited program.
  • Registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).

  • Demonstrated proficiency in individual, family, and group therapy modalities.
  • Experience with harm reduction interventions, including safer injection practices and overdose prevention.
