About Élisabeth Bruyère Residence

Élisabeth Bruyère Residence is a 69 bed Long-Term Care (LTC) home situated within Elisabeth Bruyere Hospital. We are a designated francophone organization and are proud to serve clients in both official languages. We value our employees and their professional development. Employees can participate in training the next generation of students as preceptors and participate in committees, research and innovation projects, dedicated to continuous improvement. Our systems are fully automated, and documentation is done through PointClickCare and PointOfCare. What would my role be?

Reporting to the Clinical Manager, you will provide assessment, treatment, and evaluation of individual, interpersonal, and societal needs using social service work knowledge, skills, interventions and strategies, to assist individuals and families, to achieve optimum social functioning. You will also:

  • Enhance residents' well-being and quality of life by promoting and respecting the dignity and individuality of each resident to support self-determination, independence, while supporting significant others in the patient/resident's life and assisting families in advocating needs.
  • Promote continuity of care for residents of long-term care, through the identification and use of services available in the community.

How do I qualify?

Required:

  • Two (2) year community college diploma from an accredited Social Service Worker Program
  • Minimum of one (1) year of working experience in a hospital or long-term care and gerontology setting
  • Membership in the Ontario College of Social Workers and Social Service Workers
  • Relevant experience with the elderly, behavioral disorders, palliative care convalescent care
  • Fluent in French and English (Candidates who do not meet this language requirement are eligible to apply for this position and may be considered if there are no other suitable applicants who meet this requirement).

  • Demonstrated strong interpersonal skills.
  • Demonstrated excellence in communication (both oral and written)
  • Able to be organized, to set priorities, and to work independently.
  • Demonstrated problem-solving skills, crisis intervention skills, time management skills
  • Able to participate actively and effectively as a member of the interdisciplinary team.
  • Extensive knowledge of community resources
  • Able to navigate digital resources (e.g., Health Partner Gateway portal, PointClickCare, Electronic Patient Records, and other software/websites).
  • Demonstrated knowledge of issues encountered by persons with some form of dementia or physical challenges.
  • Basic computer skills
  • Basic knowledge of medical terminology
