Social Worker II - Outreach TeamWe are seeking a highly motivated and experienced Social Worker to join our dynamic Outreach team. In this role, you will play a vital part in providing comprehensive support and resources to individuals experiencing mental health challenges within the community.
Responsibilities:- Collaborate effectively with community partners, including healthcare providers, shelters, and social service agencies, to ensure seamless access to care for clients.
- Conduct thorough risk assessments and develop individualized crisis intervention plans to address immediate needs and promote safety.
- Prepare comprehensive bio-psycho-social assessments, treatment plans, and discharge summaries that reflect a holistic understanding of each client's situation.
- Educate community partners on mental health issues, treatment options, and recovery strategies to enhance their capacity to support individuals in need.
- Provide short-term counseling, education, and follow-up services to clients, empowering them to navigate challenges and achieve their goals.
- Maintain a current understanding of social work principles, ethical standards, and best practices through ongoing professional development.
- Participate in peer consultation groups, departmental meetings, and committees to contribute to the advancement of social work practice within The Royal.
- Engage in teaching, education, and supervision activities for social work students, fostering the next generation of mental health professionals.
- Adhere to hospital policies, procedures, and relevant legislation to ensure compliance and ethical conduct.
- Maintain accurate records and documentation in accordance with established protocols.
- Master's degree in Social Work from an accredited program.
- Active registration with the Ontario College of Social Workers & Social Service Workers (OCSWSSW).
- Minimum of two to three years of clinical experience in a mental health and substance use setting.
- Demonstrated expertise in crisis intervention, assessment, consultation, and addiction counseling within a community-based context.
- Comprehensive knowledge of relevant legislation, including the Mental Health Act (MHA), Personal Health Information Protection Act (PHIPA), and The Health Care Consent Act.
- Familiarity with local community resources and support networks.
- Strong collaborative skills and experience working effectively with diverse stakeholders.
- Excellent communication, advocacy, and conflict resolution abilities.
- Proven ability to work independently and as part of a team.
- Proficiency in electronic health record systems for clinical documentation.
- Valid driver's license and access to a reliable vehicle.
- Bilingual proficiency (French/English) is mandatory. English level A- is mandatory in oral expression/comprehension, reading comprehension, and written expression. French level A- is mandatory in oral expression and oral comprehension.
